On site Licensed Clinical Social Worker

PHCC has 2 full time dentists. One at each center who offer general dental care to all ages including:

• Cleanings and preventive care
• Fillings
• Extractions
• Dentures and other care may be provided when determined appropriate as part of treatment plan

PHCC has an extensive diabetes support program. The medical provider checks urine and blood levels. The provider also performs a foot exam. Then a personalized diabetes nurse helps set self management goals, and organize free diabetic nutrition classes. The nurse also helps patients find affordable diabetes supplies.

Medical care to all ages, with special emphasis on adults and the elderly including:

• Acute medical problems
• Chronic medical problems, including a specialized diabetes program
• Annual preventive check-ups for men and women
• Employment physicals and drug screens
• Disability and worker's compensation examinations

• All childhood immunizations
• Adult immunizations
• Flu vaccine
• Pneumonia vaccine

Help in obtaining low cost prescription medicines provided to low-income and uninsured patients who qualify financially is available to individuals who are established as Primary Health Care Center patients.

Medical care to newborns, infants, children and adolescents including:

• Well-child check-ups
• Immunizations
• Acute medical problems
• Chronic medical problems

We provide pap exams, clinical breast exams and family planning. We also partner with Memorial Hospital and Susan G Komen to bring a mobile mammogram bus to provide digital mammograms at both our Rossville and Trenton centers each month. Battlefield Imaging also partners with PHCC to provide mammogram appointments to all of our female patients.
